MANTAR ENFEKSİYONLARI VE ANTİFUNGAL İLAÇLAR İLE İLGİLİ ÖNEMLİ BİLGİLER
Mantarların neden olduğu bulaşıcı hastalıklara mikoz denir; çoğunlukla kroniktir. Mikotik enfeksiyonlar cildi, cilt altını tutabilir veya sistemik enfeksiyona sebep olabilirler.
Antifungal ilaçlar, lokal veya sistemik mantar (fungus) enfeksiyonlarının tedavisinde kullanılan ilaçlardır.
Mantar hücreleri, prokaryotik bakteri hücrelerinin aksine memeli hücreleri gibi ökaryotiklerdir. Bu nedenle antifungal ilaçların mantar ve memeli hücreleri arasındaki seçiciliği düşüktür; bu da antifungal ilaçların antibakteriyel ilaçlardan daha toksik olduğunu göstermektedir.
Mantarlar antibakteriyel ilaçlara; bakteriler de antifungal ilaçlara dirençlidir.
Mantar enfeksiyonları (mikozlar) tedavi bakımından üç gruba ayrılmaktadır:
1. DERMATOFİTOZLAR VE DİĞER KÜTANÖZ (YÜZEYEL) CİLT MANTAR ENFEKSİYONLARI
- Kütanoz (yüzeyel) cilt enfeksiyonlarına neden olan küf-benzeri mantarlara dermatofit veya tinea denir. (Epidermophyton, Trichophyton, Microsporum)
- Tinea enfeksiyonları, etkilenen bölgeye göre adlandırılır (tinea pedis-ayaklardaki enfeksiyon, tinea capitis-saçlı deri, tinea corporis-vücut, tinea barbae-sakallı deri, tinea unguium- tırnak,..)
- Ciltte stratum corneumu tutarlar.
- En sık görülen enfeksiyonlardır; saç, cilt, kıl ve tırnaklarda yerleşim gösterir.
2. MUKOKÜTANOZ MANTAR ENFEKSİYONLARI
- Bu enfeksiyonlar başlıca Candida türü mantarlar (en fazla C. albicans) tarafından oluşturulur.
- Candida türü mantarlar daha çok, ıslak kalan cilt bölgelerini ve mukozaları tutarlar (Parmak araları, ağız boşluğu, bağırsak, anüs çevresi, vulvovajinal bölge gibi).
- Genellikle diyabet, uzun süreli geniş spektrumlu antibiyotik veya glukokortikoid kullanımı gibi predispozisyon oluşturacak durumu olanlarda daha sık gözlenir.
- Orofarengeal kandidiazis veya kronik mükokütanoz kandidiazis şeklinde görülebilir.
- Kronik mükokütanoz kandidiazis, az görülür ve immünsüpresif hastalarda ortaya çıkar. Ayrıca ilaçla tedaviye en dirençli kandidiyazis türüdür.
3. SİSTEMİK MANTAR ENFEKSİYONLARI
- Yerleşme alanına göre derin ve ciltaltı (hipodermik) olmak üzere ikiye ayrılır.
- Genelde az görülürler; daha çok immün yetmezliği olan kişilerde görülürler.
- En sık kandidiyazis ve aspergilozise rastlanır.
- Derin sistemik mikozlar, aspergilozis, blastomikozis, kriptokokozis, koksidioidomikozis, parakoksidomikozis, mukormikozis ve histoplazmozistir.
- Ciltaltı sistemik mikozlar, başlıca sporotrikozis, kromomikozis ve misetomadır.
